in need of 6 months diet history sample

I JUST FOUND THAT I WAS PARTIALLY DENIED I HAVE AETNA HMO....AETNA WANTS 6 MONTHS DIET HISTORY I NEED HELP I AM A FORMER MILTARY WIFE AFTER MY DIVORCE IN 2000 MY EX HUSBAND DETROYED ANYTHING OF MINES THAT HE GOT HIS HANDS ON.WE LIVED IN OKINWA JAPAN FROM 1989 TO 1992 ANY AND ALL OF MY DOCUMENTED WEIGHT AND DIET HISTORY WILL BE THERE IN MY MEDICAL RECORDS. I don't have a sample, but suggestions for what you would include in it. List any medications, diets, exercise plans, programs that you participated in (Weight Watchers, Jenny Craig) that you've done / taken. Include the start and end dates (month and years would probably suffice if you don't have exact information). Include your starting and ending weights, if you had any side effects (medications - heart palpitations, diets - excessive constipation, things like this). Maybe include a brief summary on how it was too hard to sustain or what problems you had with it. You can't stay on a grapefruit diet forever, as an example. This is all I can think of. Your medical records are just that ... yours. You should be able to get a copy from the military hospital system. Good luck. Worse comes to worst, then sit down and just start writing down a time/diet chart. The dates and results don't have to be accurate perse, just get it down on paper. Make sure you keep a copy as you will be asked for it over and over and over and over again. Good Luck.

I also have aetna, and needed the 6 month doctor supervised diet. The paperwork for the diet cannot be sent in from you, it has to be what the doctor documented during the 6 months you seen them for the diet. This 6 month diet also has had to have taken place within the past 2 years. I was also told by my surgeons office that if aetna does not like the way the pcp documented the diet, I might have to start over. You know that bit of news did not sit well with me. I'm still waiting for my surgeons office to get all required paperwork to submit to the insurance, and keeping my fingers crossed. You definately should be able to get copies of your medical records.
