Have you gone to their website...securehorizons.com There is three plans for the medicare advantage plan and each one is different. I had Medicare and my surgeon cost me $198.24 and the doctor that put me to sleep was $77.99 and I don't know what the hospital was because I haven't gotten a bill yet...it was considered day surgery. Does your book tell you the co pays for surgeries? The surgeon might know.. Good Luck...I had mine 4-16-08 and I am very glad I did it, should have done it sooner...

Medicare paid for my surgery, but the copay was for the instay hospital visit for 4 days was $876 (my 20%). There were other bills that medicare paid 80% and I the 20% which was total about $700. My surgeons bill was $124
