Does anyone have an example PCP Letter for me to send to my PCP for medically

Cindy, I have a letter of medical necessity I wrote for my PCP to my surgeon AND my insurance carrier, but it's not on a template or a disc. Feel free to use my wording and be creative by changing it around to suit you. Go to my profile and see if you can or want to use it. Then, if not, you can wait to see what some of the other posties respond. I typed my own, and took it to my PCP and asked him to sign it. I had a COMPLETE copy of ALL of my medical records that documented every diet I'd tried for over 15 years and the success. So, you know he HAD to sign it! LOL!! I think that's why everything has worked out for me. Hope it helps you and you have much success with your WLS if you're still pre-op. Let me know how it turns out!
