Question:
In a revision from a roux en y gastric bypass to a ds, is the pylorus restored?

In a gastric bypass the pyloric valve is removed. One of the benefits of DS is having a pyloric valve. If a revision is done is it somehow possible to restore the pylorus?    — hoping4arealchange (posted on September 3, 2010)

From what I understand about this procedure, they would need to reconstruct your stomach back to it's pre-RNY condition, thereby restoring the pylorus or pyloric valve and then from there they would do the DS. I know one person who has had it done but it's not the easiest route to take. It does involve a lot in terms of surgery procedures.

Teh type of revision I have heard of involves jsut redoing the size of the existing pouch to hold less and close better. This is a fairly simple procedure but they do not reconnect the old stomach. I am not sure that is possible.

It is possible, and there are several posters on the DS board who've had a RNY to DS revision. It is imperative that your remnant stomach be restored during the revision, otherwise you will not have a DS. You will have an ERNY, which is considered by many to be the worst of both worlds. There are only three surgeons in the US you should consider for a surgery like this - John Rabkin in CA, Ara Keshishian in CA, and David Greenbaum in NJ. Your pyloric valve should not have been removed during your RNY, so theoretically restoration is possible. Sometimes (depending on the length of time you've had the RNY) it takes a while for the pylorus to "wake up" and resume normal functioning. Please post on the DS messageboard so that those who have been through a revision can help you. Based on their posting history, they will be glad to share their collective wisdom.

I would suggest going to the DS forum and asking this question. You will find there are a number of people there who have had RNY to DS revisions. They can answer this question better for you. They CAN restore your paloric valve if you go to a good revision surgeon. Check out the DS forum.
