Question:
What information should I give to my Insurance Co

Help! My insurance sent a letter to my Doctor requesting additional information. They said autherization can not be determined at this time, to list any structured weight loss programs that were tried and failed. What is good to send them?    — [Deactivated Member] (posted on December 31, 2001)

MAKE A CHART START OFF WITH NAME OF DR OR GYM YOU WENT TO THEN WHAT WAS TAKEN OR DONE DATE START TO FINISH THEN WAIT LOSS/GAIN OR ETC.........I HAD DR SUPERVISED DIET FOR ABOUT 6 YEARS OFF AND ON THEN I ALSO HAD GONE TO LIKE GLORIA MARSHALL (GYM) CLOSED NOW.....SKINNY SCHOOL ALSO CLOSED NOW..GOOD LUCK IF YOU NEED HELP YOU CAN EMAIL ME......... It does sound very promising that they did not ask for doctor supervised diets, but rather "structured" programs. I would list EVERYTHING you have tried. Include date started and weight and then length of time in programs and weight loss. Then list the next one and be sure to include that starting weight which of course will show a gain after ending the first and so on. Include any gym memberships and length of time in program. Include any self-diets, also. (atkins, cabbage soup, etc...) If you have any receipts or cancelled checks for weight watchers, gym membership,etc... include those. Most insurance companies insist that these diets be doctor supervised, but with the wording of your company's request, I believe that they are being less strict. Hope this helps!!!
