I've heard that your employer can choose to give you a "rider" or some such that allows YOU to be covered for the particular procedure. Its sort of a case by case thing and lays in the employers hands, and I would guess costs them something. Maybe if you have a lot of comorbids and could make a great case for yourself using lots of letters from doctors and specialists as to how this could help or save you then you could present it to your employer. If that fails, changing jobs to somewhere that carries insurance that will pay is an option. As for individual policies, in my experience if your weight is high enough and/or you have enough comorbids to qualify for WLS then you're not likely to find an insurance company who'll accept you for any quality individual insurance plans.
