Question:
Does anyone have a copy of the letter from their PCP

My PCP is willimg to help me, but she is asking for a sample letter that another PCP wrote... I was denied for surgery,this is my second appeal and I believe this letter would be most helpful. thx.    — jazzaie (posted on October 12, 2003)

Hi karen, I'm also in the appeals process. It is a bit discouraging when Dr.'s are willing to write a letter, but then it's only 3 lines :( I found a book called " The Dr.'s Guide to Weight Loss Surgery" and in there is a sample letter, I made copies of it to give to my Dr. so he would know what they expected. I don't know how to attach and send thing on the computer, or I would just send it to you. I hope this helps you. Hang in there!!!!!!!!! Christy
