Question:
examples of diet history

I was wondering if anyone had an example of what my diet history letter is supposed to look like. I have to get it done and have no idea how I am suppose to write it. I understand that it needs to be five years and that I need to put wieght lost and gain back but I need a format to put it in. thanks    — amy H. (posted on November 11, 2003)

Hi Amy! Well, I have a copy of my diet history on file, so I can either email it to you, or put it in the mail so you can get an idea.
