Question:
Help I need documentation of diets.

My surgeon said Cigna hmo requires documents proving you were on diets for at least 12 weeks I was in weight watchers but it was so long ago does anyone have general documents for this program that will be good enough for me to send to Cigna it has to track my weight in a 12 week period I can fill it out if anyone has a form. I was faced with the same problem. I've been on dozens of diets in my life, but who keeps records? I drew up a list of the diets/programs I could remember, and made up the details. Nobody ever said a word about it. How are they going to know?

Hi, If your insurance insists on documentation, then you may have to do a 12 week program of some kind. You can set a program up with your dr. You will probably have to weigh in every 2 weeks or so. But it will be worth it if it helps you get approval from insurance. Good Luck.
