I need a letter for pcp for medical supervised diet

My doctor told me I could write up the letter and he would sign it I took Phen Phen under his care I need a sample letter of what I should include in letter so the insurance company will except this as a medically supervised diet. Thanks    — Donna B. (posted on May 15, 2002)

That will really depend on what your insurance company will accept---I have been a diabetic for 15 years and BCBS would not accept a letter from my PCP stating that he had been treating me for the last 7 years for diabetes and had monitored my diet for the last 7 years---they wanted a month by month weigh in --proof of an office visit for weigh ins--which they do not cover the cost of---go figure
