Does your doctor realize you just need this for the insurance? If so, and he/she still doesn't want to do it, then I'd try another doctor.

You need to go see a Doctor that knows about Gastric Bypass, and one that fully supports your decision. Because he/she will be the one following you after your surgery. He/She need to know which medications you can and can not take. How much to decrease your current medication if any. And you need to trust this person. Good luck :)

Ask the PA to write the letter for you. I am a Nurse Practitioner and I write letters to insurance companies all the time and have never been rejected, b/c I am not a Dr.
