Question:
My insurance (health assurance ppo ) want me to provide a letter of past diets

I went to a medically supervised diet center. That was Feb.01. Lost some weight and stop May 01. Gained back all the weight and more. My insurance want recent weight loss attemps within the last 6 month. My PCP is very strick and does not promote anything , but counting calories. What should I do. I could go back to the weight loss center, but well this be enough. My appointment well be in June with the doctor I want the surgery with. I have heard that most weight-loss centers don't keep records of past members. Perhaps you could come up with cancelled checks that you wrote to the center? Maybe you still have your progress booklets or paperwork from them? Try submitting those things, or copies of them, as proof of enrollment. I would really try to come up with anything you can think of as proof. Maybe even a good letter from a close friend or family member who knew you were attending a weight-loss center...certainly couldn't hurt if you have nothing else to submit.
