1. go to MY OH

2. go to MY ACCOUNT

3. sign in with your user name & password

4. in the middle section, under SURGERY RELATED, click on "I'VE GOT A DATE"

5. enter your date and info, scroll to the bottom, SAVE MY WORK

That's it.
