PS...under surgery, this does not necessarily mean that that is the ONLY type of surgery that surgeon performs, that is just the info that was posted by the patient.

I am also Dr. Averbach/Lap Band and my fees were the same - $650 out of pocket. I have United Healthcare.

Highest Wt: 274 / LAP-Band Low: 180 / Sleeved at 233 / Goal: 160!

I am feeling technologically incapable, so I didn't put it into table form, but you could add my dr. to the list

Dr. Afram: Open RNY, George Washington University Hospital (its a Center of Excellence), $1500 program fee
I think they work with many insurances, but I "know" they have a contact at BC/BS (Care First)
