Need info plzzz :)

I have Medicaid share of cost. I don't have a card to prove it so what do I need to bring with me to my visit with Dr hoddinott?

my apt is sept 27th, I called them and made the apt so I don't have a referral from a PCP. Please help

call medicaid they will send you a new card plus if you have a certification letter that you receieved from medicaid with your with your approval date that will help they just need to be able to a  medicaid number or some one to contact to get approval of benefits. i hope this info can help ps. you can also call children and family for help with this situation the new card should be to you before your app with dr.
